Sample Military Feedback Forms

The military does many things well. In my experience, one of the things they do exceptionally well is providing regular, substantive feedback to its members on their performance. The discipline with which they do so (on a quarterly basis) far exceeds my experience in the corporate world. As I’ve often been asked for suggestions about how to provide substantive feedback to veteran hires, I’d suggest you review the means by which they are used to receiving feedback on a regular basis. I’m attaching two versions of the Department of the Army Form 4856, which they call a Developmental Counseling Form. One is more dated than the other, just to show you how the form has evolved and to provide multiple examples. Supervisors in the military use this (and versions thereof for application in other Services) on a quarterly basis to provide formal feedback to their charges. If you don’t have formal feedback processes in place in your organization, you might consider using these as a means to kick-start your efforts to do so. As with everything else I recommend, leverage your in-house veterans and/or veteran affinity group in its implementation.
